The food hygiene rating is not an opinion. It is calculated from three scores taken at the inspection, and because the arithmetic is published, a business can work out in advance roughly where it would land. Almost every business that is surprised by its rating was surprised by the third score rather than the first two.
The three components
Hygienic food handling, the condition of the structure and equipment, and confidence in management. Each is scored, and the rating comes from the total together with the worst individual score, so one bad component caps the result however good the others are.
Why management is the one that moves
Handling and structure are visible and most owners already manage them. Confidence in management is about the documented system, the records, and whether the person in charge can explain them. It is the component most often lost and the cheapest to recover.
The checklist that matters
The food safety management system in use rather than in a drawer, temperature and cleaning records completed at the time, allergen information available and accurate, staff able to describe their own checks, supplier and traceability records, and pest control evidence.
Getting to five
Fix the structural items that were flagged, then put the records right and keep them right for long enough that the pattern is real. Where the business has already been rated, a request for a re inspection after the work is done is the route back, and an independent audit first is how businesses avoid asking too early.

Are ratings published?
